A 19 yr old male with c/o of vomitings & SOB

 Past admission  18 year old male patient elog June 12, 2021  A 18 year old male,from miryalaguda,who is a student, came to the hospital with chief complaints of low backache 1 week ago,fever since 5 days ,yellowish discolouration of eyes since 3 days ,vomitings (2 episodes) and loose stools(3 episodes)and blood tinged urine yesterday morning HISTORY OF PRESENT ILLNESS- Patient was apparently asymptomatic 6 months ago,then he noticed gradual loss of weight since 6months,patient had history of polyuria,nocturia,polydypsia since 2 months 10 days ago, patient attended a function outside and after 2 days he developed low backache and 2 episodes of vomitings and 3 episodes of loose stools for one day which subsided on its own. Next day,patient developed fever,intermittent,high grade,subsided with medication. Patient went to RMP and got treated for fever.patient noticed yellowish discolouration of eyes and urine 3 days ago.Nausea and loss of appetite + Burning micturition is present PAST HIS

NEW ADMISSION  AMC BED 6  A 19 yr old male student came with c/o b/l leg pain since 16/3/22 midnight  In 2008 pt had yellowish discoloration of eyes ,visited our hospital, initially guven iron supplements i/v/o anemia, Hb was around 7 gm /dl In 2012 pt was adviced for Hplc - which showed      Patient HbF :27.7 % ,HbAo:5.4% ,HbA2 - 4.7 ,HbS -61.3 diagnosed with sickle cell and beta thalassemia trait(i/v/o   increased HbA2 ).     Father HbF : 2.8 ,HbAo:82.7, HbA2: 4.9,HbS -diagnosed with thalassemia trait

This E log reflects my patient centred online learning portfolio and your valuable inputs in the comments section. 40year old male  who works as a field assistant who is chronic alcoholic came to casualty with complaints of abdominal pain since one day & vomitings (10-15 episodes)  since one day   Patient was apparently asymptomatic 1 day back  , since 3 days there h/o binge alcohol consumption (360ml whisky)   after which since 1 day he developed pain abdomen which was squeezing type predominantly in epigastric region diffuse type associated with nausea & vomiting 10 to 15 episodes/day.
